Once you’ve installed the plugin Revolution Slider you can create any no. of sliders.

Once you have created a contact forms. Just go to the page where you would like to add contact form(s) and follow the below steps,

Step 1 : Click the Backend editor at the top left above the editor.

Step 2 : Now, choose Revolution Slider 5 module/element from the boxes.

Step 3 : You must see the Revolution Slider settings. just choose created slider from the dropdown.
